Medical forms MUST be uploaded prior to our first practice on October 13, 2026 into MyCAK-Resources- August Schools: Student Medical Forms- Athletic Packet. ** Note, if your child participated in the spring physicals onsite at CAK, a digital copy of their physical was uploaded into their profile that you can view.

All physical exams must be completed after 4/15/25 to be valid for the 25'-26' school year. THE CONSENT FOR ATHLETIC PARTICIPATION AND MEDICAL CARE PAGE MUST ALSO BE UPLOADED WITH THE ANNUAL PHYSICAL.
The two concussion forms and sudden cardiac arrest forms must be submitted to be cleared for participation.

I have included the slide deck from our info meeting on September 2nd where many of your questions can be answered. As more things are solidified, we will communicate them but please don't hesitate to reach out with questions! We are so excited for this season! Go Warriors!
